To register a dependant, you can:
- Download the Application to Register Dependents form from the SAMWUMED website.
- Collect the Application to Register Dependents form at your nearest HR Department.
Complete the form fully, including all relevant information for your dependant. Attach the required documents and submit the form to your allocated HR Department for approval. Once approved (stamped), email the form to memberupdates@samwumed.org for processing.
Waiting Period:
The Scheme generally imposes a three-month waiting period for dependants who have not been part of any medical aid scheme previously. Additionally, a twelve-month waiting period may apply for any pre-existing conditions.
Exception: If you belonged to a previous medical aid for 24 months or more without a break in cover exceeding 90 days, immediate benefit cover will apply (meaning your dependant will be covered immediately).
