OPTION A

Paid from Capitation Benefits.

Option A Benefits

  • One composite eye test per beneficiary per benefit cycle is limited to R755 at a Network Provider and R400 at a Non-Network Provider.
  • One pair of clear single-vision spectacle lenses limited to R215 at a Non-Network Provider or clear flat top bifocal spectacle lenses limited to R460 at a Non-Network Provider or clear multifocal spectacle lenses limited to R810 at a Non-Network Provider.
  • Lenses are covered in full at PPN Network Provider.
  • Frame and or lens enhancements per beneficiary are limited to R1,080 at a non-network provider and R1,350 at a PPN Network Provider.
  • There is no benefit for reading glasses or contact lenses.

Conditions:

  • Optical benefits are subject to Scheme protocols and the PPN Network.
  • Optical benefits are subject to a 2-Year cycle.
  • To access benefits members must check their claim status.

OPTION B

Paid from Capitation Benefits.

Option B Benefits

  • One composite eye test per beneficiary per benefit cycle limited to R755 at Network Provider and R400 at Non-Network Provider.
  • One pair of clear single vision spectacle lenses limited to R215 (two hundred and fifteen rand only) per lens; or One pair of clear flat top bifocal spectacle lenses limited to R460 (four hundred and sixty rand only) per lens.
  • One pair of clear multifocal spectacle lenses including branded lens add-on limited to R850 (eight hundred and fifty rand only) per lens.
  • Lenses are covered in full at a PPN Network Provider.
  • Frame and or lens enhancements per beneficiary are limited to R1,408 at non-network provider and R1,760 at a PPN Network Provider.
  • Contact lenses are limited to R3,000 per beneficiary.
  • There is no benefit for reading glasses.
